Gap Assessment

2–4 weeks

We begin with a thorough, standards-based review of your current compliance posture. Our consultants conduct document reviews, leadership interviews, tracer activities, and environment-of-care walkthroughs to produce a prioritized gap report with clear, actionable remediation steps.

Remediation Planning

1–2 weeks

Working directly with your leadership and department heads, we develop a structured remediation plan with assigned ownership, realistic timelines, and measurable milestones. We help you sequence the work to address highest-risk findings first and build momentum across the organization.

Survey Readiness

4–8 weeks

As your survey date approaches, we conduct mock surveys using the same tracer methodology surveyors use, facilitate targeted staff education sessions, review and strengthen your documentation, and ensure your leadership team is prepared to manage the survey process effectively.

Post-Survey Support

As needed

If findings are issued, we help you develop a credible, well-supported Plan of Correction and implement sustainable corrective actions. We also help you build the ongoing monitoring systems needed to maintain compliance between surveys.

How far in advance should we engage Meridian before our survey?

For initial accreditation, we recommend engaging 6–12 months before your target survey date. For re-accreditation, 3–6 months is typically sufficient, though organizations with known compliance gaps benefit from earlier engagement.

Can Meridian help if we've already received findings from a previous survey?

Absolutely. Post-survey remediation is one of our most common engagements. We help organizations develop credible Plans of Correction, implement sustainable fixes, and build the monitoring infrastructure to prevent recurrence.
